General inquiry privacy notice

What we collect

We collect your name, email, optional phone, city and state, service interest, preferred contact method, and optional message. Employer inquiries also include company name and estimated monthly testing volume.

How it is used

We use these details to respond, discuss requirements, and prepare a quote. General contact details and inquiry source may be recorded in HubSpot for follow-up. Employer service interests may also be recorded. Free-text messages and individual testing interests are not sent to HubSpot. Do not include Social Security numbers, medical information, donor details, or test results.

Storage and communications

Inquiries are stored privately and are not readable by website visitors. Access by authorized personnel is limited to handling your request. Ordinary email is not a secure testing intake channel. We do not claim that a general inquiry form or a future embedded intake form makes this website HIPAA compliant.

Marketing and privacy requests

Submitting an inquiry is not marketing consent. To ask about your information or request deletion, email testing@kghealthgroup.com. No retention period is stated until a reviewed policy is confirmed.
